PFT Manager, Residential Programs – 2025-018
The Manager, Residential Programs, is responsible for the leadership, development, and oversight of CMHA HPE’s residential programs. Our programs provide transitional housing and support services to individuals experiencing homelessness or housing instability due to mental health and/or addictions challenges. Reporting to the Director, the Manager ensures the program is client-centered, evidence-based, and aligned with CMHA HPE’s strategic priorities. As a key member of the Leadership Team, the Manager oversees service delivery, ensures program compliance with relevant legislation and funding agreements, and fosters relationships with community partners to enhance housing stability and mental health outcomes. TFT Clinical Counsellor / Psychotherapist – 2025-009
The Clinical Counsellor/Psychotherapist, reporting to the Clinical Practice Lead, is responsible for the provision of evidenced based psychotherapeutic assessment and treatment to individuals 16 years of age or older with cognitive, emotional or behavioural disturbances through a therapeutic relationship based primarily on verbal or non-verbal communication, in individual or group session formats. Further to this, duties will include participation in clinical case consultation, multidisciplinary team treatment planning meetings, psychoeducational groups, provision of student placements/supervision, community liaison, and community presentations as required. PPT Personal Support Worker – 2025-012
The Personal Support Worker (PSW) is a member of the multi-disciplinary team at our Back-to-Home Program (BTH). The PSW provides personal support and activities of daily living support to residents of the program. The PSW provides services that directly support residents to adequately maintain their living environment and personal care activities while promoting learning and skills development. The PSW collaborates with members of the BTH multi-disciplinary team in meeting the needs of residents to attain an optimal level of independence and wellbeing. PFT Registered Practical Nurse (RAAM Clinic) – 2025-016
As a Registered Practical Nurse (RPN) at our Rapid Access Addictions Management (RAAM) clinic, you will play a vital role in delivering compassionate and effective care to patients who wish to address problematic substance use. The Registered Practical Nurse (RPN) at our Rapid Access Addiction Medicine (RAAM) Clinic will support the provision of low-barrier addictions medicine care to individuals with substance use disorders in a variety of settings, including in office and in the communities of Hastings and Prince Edward Counties. TFT IMPACT Co-Response Worker – 2025-017
The IMPACT Co-Response Worker (ICW) works as part of a multidisciplinary team and under the guidance of the IMPACT Program Manager and will provide mobile addiction and mental health (AMH) support and services in collaboration with local police services. Embedded in police service locations, the ICW will assist with AMH-related calls for service when appropriate, acting as a liaison between police and the AMH health system, including crisis intervention and primary care services. PFT Youth Substance Use Addictions – 2025-020
The Youth Substance Use Addictions Counsellor works as a member of a multi-disciplinary, multi-agency team to provide community-based support and treatment services, including withdrawal support, to individuals aged 16-24 years as part of an integrated community substance use program for youth. Duties include screening and assessment to ensure the most appropriate interventions are offered to the client including community or residential withdrawal and treatment services, negotiating treatment goals and a treatment plan, case management, personal/supportive counselling, liaising with Primary Care, community-based youth services and residential withdrawal services, and facilitating internal/external referrals. The Mental Health Case Manager (“MHCM”) is responsible for providing case management services to individuals with a serious mental illness who are living in the community. TFT Mental Health Case Manager (Back to Home) – 2025-022
The Back to Home Case Manager (BTH CM) works as a member of a multi- disciplinary team and is responsible for providing the full range of case management supports and services including screening, assessment, and treatment planning to ensure the most appropriate interventions are offered to BTH clients, with a caseload of 18-25. The BTH CM will provide individualized treatment planning, negotiate client focused treatment goals, and group facilitation. This position works in close partnership primarily with acute care hospitals, long-term care, community agencies, primary care, support systems, and other service providers, ensuring client voice and choice and seamless transitions.
