The peer mentor is an individual with lived experience providing direct service, developing and maintaining supportive relationships with clients and acting as a role model. Duties may include: developing plans to assist, instruct and monitor clients in their personal recovery as it relates to household management including grocery shopping, budgeting, banking, errands, laundry, household chores and home safety; facilitating recovery as it relates to daytime activities including transportation and accompaniment to social/recreational and leisure activities; assisting in accessing community resources; bringing a consumer perspective to the worker/client relationship and to staff of the agency; managing and effectively administers caseload requirements. The Addictions Counsellor – Community Youth Substance Use Support & Treatment Program works as a member of a multi-disciplinary, multi-agency team to provide community-based support and treatment services, including withdrawal support, to individuals aged 16-24 years as part of an integrated community substance use program for youth. Duties include screening and assessment to ensure the most appropriate interventions are offered to the client including community or residential withdrawal and treatment services, negotiating treatment goals and a treatment plan, case management, personal/supportive counselling, liaising with Primary Care, community-based youth services and residential withdrawal services, and facilitating internal/external referrals. PFT Addictions Supportive Housing (ASH) Case Manager – 067

The Addiction Supportive Housing (ASH) Case Manager is responsible for providing comprehensive outreach case management services to individuals within the Addiction Supportive Housing Program. This position works in close partnership with community service agencies to provide assessment, intervention, supportive counselling, service coordination and follow-up to adults, 16-years of age and older within the ASH Program. The Addiction Supportive Housing Case Manager is responsible for providing services to a minimum caseload of 8, unless otherwise directed, on an ongoing basis until discharged from the ASH Program. PPT Registered Practical Nurse (0.6) – 068

The Back to Home (BTH) Registered Practical Nurse (RPN) is responsible for providing quality care to residents participating within the Back to Home Program and providing leadership to others as assigned. The Back to Home Program is an innovative, clinical wrap around care, multi- disciplinary, therapeutic, and rehabilitative supportive housing program for individuals living with mental health/and or addictions (MHA) issues, potentially with co-occurring medical or physical health needs. In collaboration with the BTH team, the RPN will be responsible for supporting the residents by working collaboratively to develop the treatment plan, providing medical, physical, social, and mental support as required and as per CMHA HPE standards.
