PPT Residential Counsellor 1 (24/7 Supportive Housing, Weekend Days) –

The 24/7 Supportive Housing Residential Counsellor 1 (RC1), reporting to the Residential Supervisor, works as a member of a multi-disciplinary team to provide ongoing side-by-side in-house and community support, Activities of Daily Living teaching and stabilization for individuals living with a serious mental illness, addictions issues, dual diagnosis, and/or concurrent disorders. Working collaboratively with individuals, family members, team members, treatment team members and other service providers, the incumbent provides services within a supportive housing model, aiming for individual self-sufficiency to encourage the potential for clients to transition to a more independent living situation. PFT Crisis Bed / Housing Case Manager – 2025-002
The Crisis Bed/Housing Case Manager is responsible for providing case management services to individuals who have been referred to the Short-term Residential Crisis Services (“SRCS”) program, individuals who live at our 24/7 Supportive Housing program, residents in our Home For Good units and other individuals as appropriate. This position works in close partnership with 24/7 Supportive Housing Residential Counsellors, Hastings County Housing, acute care hospitals and community service agencies to provide assessment, intervention, supportive counselling, service coordination and follow-up to adults, 16-years of age and older. PPT Mental Health Case Manager (Bancroft) – 2025-003
The Mental Health Case Manager (MHCM) is responsible for providing case management services to individuals with a serious mental illness who are living in the community. This position works in close partnership with acute care hospitals and community service agencies to provide assessment, intervention, supportive counselling, service coordination and follow-up to adults, 16-years of age and older.
