TPT Clinical Counsellor / Psychotherapist (0.5 FTE) – 2022-99
The Clinical Counsellor/Psychotherapist is responsible for the provision of psychotherapeutic assessment and treatment to individuals 16 years of age and older with cognitive, emotional, or behavioural concerns/issues through a therapeutic relationship based primarily on verbal or non-verbal communication, in individual or group session formats. The Back to Home Program is an innovative, clinical wrap around care, multi- disciplinary, therapeutic, and rehabilitative supportive housing program for individuals living with mental health/and or addictions (MHA) issues, potentially with co-occurring medical or physical health needs. PPT Mental Health Case Manager (Belleville) – 2023 – 026
The Mental Health Case Manager works as a member of a multi-disciplinary team and is responsible for providing case management services to individuals with a serious mental illness who are living in the community. This position works in close partnership with acute care hospitals and community service agencies to provide assessment, intervention, supportive counselling, service coordination and follow-up to adults, 16-years of age and older. PPT Residential Counsellor I (Weekend, Days) – 2023 – 027
The 24-7 Supportive Housing Residential Counsellor provides ongoing side-by-side in-house and community support, Activities of Daily Living teaching and stabilization for individuals living with a serious mental illness, addictions issues, dual diagnosis, and/or concurrent disorders. Working collaboratively with individuals, family members, team members, treatment team members and other service providers, the incumbent provides services within a supportive housing model, aiming for individual self-sufficiency to encourage the potential for clients to transition to a more independent living situation.
