The Addiction Supportive Housing (ASH) Case Manager works as a member of a multi-disciplinary team to provide comprehensive outreach case management services to individuals within the Addiction Supportive Housing Program. This position works in close partnership with community service agencies to provide assessment, intervention, supportive counselling, service coordination and follow-up to adults, 16-years of age and older within the ASH Program.
